ABSTRACT

Objetivo: verificar o conhecimento de acadêmicos de Odontologia de uma instituição de ensino superior da Paraíba acerca da avulsão dentária. Métodos: foi realizado um estudo transversal, no qual 64 acadêmicos responderam um formulário com perguntas objetivas relativas a dados sociodemográficos, período de formação do curso e conhecimento e condutas em casos de avulsão dentária. Foi realizada análise descritiva de frequência absoluta e relativa dos dados (SPSS, v. 20.0). Resultados: a maioria dos acadêmicos recebeu informações sobre avulsão dentária em aulas ministradas no curso (86%) e indicaria a irrigação com soro fisiológico seguida de reimplante quando da ocorrência do trauma há menos de uma hora (64,1%) e há mais de uma hora (43,8%). O tratamento endodôntico foi indicado, independentemente do tempo do dente fora do alvéolo, por 34,4% da amostra. A contenção rígida foi a mais indicada para o dente reimplantado (48,4%) e seu tempo mínimo de proservação radiográfica respondido pela maioria dos pesquisados foi de 6 meses (48,4%). Conclusões: apesar de a maioria dos acadêmicos ter recebido informações acerca da temática, o conhecimento foi considerado insuficiente em relação a condutas referentes ao reimplante dentário, indicação do tratamento endodôntico, tipo de contenção e tempo de proservação.


Objective: to verify the knowledge of dental students from a Higher Education Institution of Paraíba about dental avulsion. Methods: a cross-sectional study was carried out, in which 64 students answered a form with objective questions regarding sociodemographic data, course period, knowledge and conduct in cases of tooth avulsion. Descriptive analysis of absolute and relative frequency of data was performed (SPSS, v. 20.0). Results: most students received information about dental avulsion during the graduation classes (86%) and would indicate irrigation with saline solution followed by reimplantation when the trauma occurred less than one hour (64.1%) and more than one hour (43.8%). Endodontic treatment was indicated, regardless of the time the tooth was out of the dental socket, according 34.4% of the sample. Rigid retention was the most indicated for the reimplanted tooth (48.4%) and six months was the minimum radiographic follow-up time answered by most students (48.4%). Conclusions: although most students have received information about the topic, the knowledge was considered insufficient in relation to some aspects of the protocols recommended for emergency care for dental avulsion, with emphasis on behaviors related to dental reimplantation, indication of endodontic treatment, type of containment and follow-up.

ABSTRACT

Abstract This study aimed to assess the traumatic dental injuries (TDIs) in permanent dentition among patients who attended at the outpatient clinic of a Brazilian dental school, during the last 20 years, and to investigate factors associated with the severity of these injuries. Clinical records of patients who attended a specialized center for dental trauma care in Brazil presenting at least one TDI in a permanent tooth, between the years 2000 and 2019, were reviewed. The data recorded were sex, age, affected arch, etiology, number, and type of the teeth affected, and classification and severity of the TDIs. The diagnosis and classification of the TDIs were based on the guidelines of the International Association of Dental Traumatology (IADT). The severity of each patient's injuries was defined as mild, moderate, or severe. Descriptive statistics, chi-square and multinomial regression analyses were used to evaluate the results. The significance level was set at 5%. A total of 837 clinical records were included, totaling 2357 teeth. Males were more prevalent than females. The patients' age ranged from 5 to 71 years. The most common traumas were avulsion (n=512) and uncomplicated enamel-dentin fracture (n=488). Univariate analyses showed that there was a statistically significant association between age group (p=0.004), etiology (p=0.000) and number of teeth affected (p=0.000) with severity of dental trauma. In conclusion, TDIs that occurred in Piracicaba and region are epidemiologically similar to those found worldwide, and that more severe injuries are related to age range, etiology and number of teeth affected.


Resumo Esse estudo objetivou avaliar as injúrias dentárias traumáticas (IDTs) na dentição permanente entre os pacientes que compareceram ao ambulatório de uma faculdade de odontologia brasileira, durante os últimos 20 anos, e investigar fatores associados à severidade dessas lesões. Os registros clínicos dos pacientes que compareceram a um centro especializado de atendimento em traumatismo dentário no Brasil apresentando pelo menos uma IDT em dente permanente, entre os anos de 2000 e 2019, foram revisados. Os dados registrados foram sexo, idade, arco dental afetado, etiologia, número e tipo dos dentes afetados, e classificação e severidade das IDTs. O diagnóstico e a classificação das IDTs foram baseados nas diretrizes da Associação Internacional de Traumatologia Dentária (AITD). A gravidade das lesões de cada paciente foi definida como leve, moderada ou severa. Estatísticas descritivas, teste qui-quadrado e análises de regressão multinomial foram usadas para avaliar os resultados. O nível de significância foi fixado em 5%. Um total de 837 registros clínicos foi incluído, totalizando 2357 dentes. O sexo masculino foi mais prevalente que o feminino. A idade dos pacientes variou de 5 a 71 anos. Os traumas mais comuns foram avulsão (n=512) e fratura não-complicada do esmalte-dentina (n=488). As análises univariadas mostraram que houve associação estatisticamente significativa entre a faixa etária (p=0,004), etiologia (p=0,000) e número de dentes afetados (p=0,000) com a gravidade do traumatismo dentário. Em conclusão, as IDTs que ocorreram em Piracicaba e região são epidemiologicamente semelhantes aos encontrados em todo o mundo, e que lesões mais graves estão relacionadas à faixa etária, etiologia e número de dentes afetados.

ABSTRACT

Objetivo: Avaliar o efeito de uma intervenção educativa, sobre avulsão de dentes permanentes, no nível de conhecimento de participantes de uma equipe do SAMU. Métodos: Participaram acadêmicos de Medicina que compunham a equipe do SAMU de Juiz de Fora (MG). A intervenção educativa foi realizada por meio de uma palestra com duração de 15 minutos. Para coleta dos dados foi utilizado um questionário, contendo 13 perguntas sobre avulsão dentária, aplicado em três etapas: antes da palestra (T0), imediatamente após a palestra (T1) e quinze dias após a palestra (T2). Foi realizada análise descritiva e teste McNemar para análise estatística (p < 0,05). Resultados:A amostra foi composta por 36 indivíduos. Nenhum participante relatou ter prestado atendimento a um caso de avulsão dentária. Trinta e quatro acadêmicos informaram não ter recebido orientações anteriores sobre o que fazer diante desse episódio e 35 afirmaram que esse conhecimento é necessário para o médico do SAMU. Houve aumento na porcentagem de acertos após os acadêmicos assistirem a palestra educativa (T1), para sete questões avaliadas. As mesmas questões também apresentaram maior percentual de acertos quinze dias após a palestra educativa (T2). Não houve diferença na capacidade dos acadêmicos reimplantarem um dente avulsionado tanto imediatamente após a palestra educativa (T1), quanto quinze dias após as orientações (T2) (T0-T1: p = 0,999; T0-T2: p = 0,999). Conclusão:A palestra educativa influenciou de forma significativa à melhora do conhecimento sobre avulsão dentária dos acadêmicos de Medicina que fazem parte da equipe do SAMU-JF.


Aim: To evaluate the effect of an educational intervention concerning the avulsion of permanent teeth upon the level of knowledge of members of a SAMU (Ambulance) team. Methods:Medical students who were members of the SAMU team in Juiz de Fora (MG) participated in this study. The educational intervention on the theme was carried out by means of a 15-minute lecture. Data collection was conducted using a structured questionnaire containing 13 objective questions on tooth avulsion, applied to the medical students in three stages: before the lecture (T0), immediately after the lecture (T1), and fifteen days after the lecture (T2). Descriptive analysis and the McNemar test were performed (p < 0.05). Results: The sample consisted of 36 individuals. All participants reported never having attended a dental avulsion case. Thirty-four students reported that they had not received previous guidance on what to do when faced with this type of episode, and 35 stated that information about dental avulsion is necessary for SAMU doctors. For seven of the evaluated questions, an increase was identified in the percentage of correct answers after the students attended the educational lecture (T1). The same questions also showed a higher percentage of correct answers fifteen days after the educational lecture (T2). No difference was found in the students' ability to reimplant an avulsed tooth either immediately after the educational lecture (T1) or fifteen days after receiving guidance on the subject (T2) (T0-T1: p = 0.999; T0-T2: p = 0.999). Conclusion: The educational lecture significantly influenced the improvement of knowledge about dental avulsion among medical students who are members of the SAMU-JF team.

ABSTRACT

To compare the viability of periodontal ligament (PDL) cells stored in Hanks Balanced Salt Solution (HBSS) with those in readily available transport media over a variable period of time. Methods: Periodontal ligament cells harvested from premolars freshly extracted for orthodontic reasons were cultured for exponential growth. The cells were exposed to egg white, evaporated milk, water and Hanks Balanced Salt Solution (HBSS) at room temperature. Their viability was evaluated after 30 minutes, 1 hour and 3 hours with the tetrazolium salt-based colorimetric assay (MTT assay). Statistical analysis was done using the IBM® SPSS version 23.0 software. Comparison between the Mean Optical Densities (MODs) of the cells stored in HBSS and other media at each time interval was done using the independent t test. Repeated measure ANOVA and Tukey post-hoc test were also carried out to compare the MOD of cells within each medium over time. The level of significance was set at p <0.05. Result: The PDL cells stored in egg white had higher MODs than those in HBSS at 30 minutes and 1 hour. Conversely, the MODs of the cells stored in milk and water were lower than those in HBSS at all the studied points. There was a significant difference in the viability of the cells stored in HBSS and water at all the time points (p<0.05). Conclusion: For up to an hour, egg white was found to perform better than HBSS in supporting the viability of PDL cell

ABSTRACT

Abstract This systematic review and meta-analysis aimed to analyze the impact of the COVID-19 pandemic on dental trauma patient attendance. The study was registered in the PROSPERO system, using the CRD42021288398 protocol. Searching was performed in PubMed, Scopus, Web of Science, Embase, Lilacs, and OpenGrey databases, using the following keywords: "Tooth injuries," "Dental trauma," "Traumatic Dental injury," and "COVID-19". We included observational studies evaluating dental trauma in the context of the COVID-19 pandemic. Quality assessment was performed using the Joanna Briggs Institute Critical Appraisal Checklist for Cross-Sectional Studies. Meta-analysis was performed in RevMan 5.4 software with Odds Ratios as a pooled measure of effect, with a 95% confidence interval, and using random-effects modeling. After applying the eligibility criteria, 32 studies were included for qualitative analysis, in which 10 were used to assess the frequency of dental trauma diagnoses in dental emergencies. Despite the decrease of visits during COVID-19, the analysis revealed no difference between the pandemic and pre-pandemic periods. Regarding the type of dental trauma, two of the studies revealed no differences for the periods before and during the pandemic. This study revealed that the COVID-19 pandemic has not impacted the frequency or type of dental trauma compared to previous periods.

Introdução: A avulsão dentária é caracterizada pelo deslocamento total do elemento dentário para fora do alvéolo em decorrência de um trauma externo e as medidas emergenciais prestadas ainda no local do acidente são imprescindíveis para um bom prognóstico. Objetivo: Investigou-se o nível de conhecimento e atitudes de leigos sobre possíveis condutas frente à avulsão de dentes permanentes. Métodos: A coleta de dados foi realizada a partir de um questionário contendo perguntas sobre características gerais da lesão e as atitudes que os pacientes tomariam em um caso de avulsão. Os dados foram analisados pelo teste de Mann Whitney, considerando um nível de significância de 5% (p <0,05). Resultados: Questionários foram respondidos por 120 pacientes enquanto aguardavam atendimento. A média geral de respostas corretas / atitudes esperadas foi de 3,74. Os participantes que possuíam informação prévia acerca do tema, obtiveram médias significativamente maiores, o mesmo não foi verificado com aqueles que possuíam experiência prévia com trauma. Conclusão: Conclui-se que o conhecimento sobre avulsão dentária dos pacientes é baixo e que a educação em saúde, através da informação da população pode ser um fator relevante para a melhora da conduta emergencial no local do trauma pela população leiga... Introduction: Tooth avulsion is characterized by the total displacement of the tooth out of the socket as a result of an external trauma and emergency measures provided at the accident site are essential for a good prognosis. Objective: We investigated the level of knowledge and attitudes of lay people about possible behaviors in the face of avulsion of permanent teeth. Methods: Data collection was performed using a questionnaire containing questions about general characteristics of the lesion and the attitudes that patients would take in a case of avulsion. Data were analyzed using the Mann-Whitney test, considering a significance level of 5% (p <0,05). Results: Questionnaires were answered by 120 patients while waiting for care. The overall average of correct answers/expected attitudes was 3.74. Participants who had previous information on the subject had significantly higher averages, the same was not verified with those who had previous experience with trauma. Conclusion: It is concluded that the knowledge about dental avulsion of patients is low and that health education, through population information can be a relevant factor for the improvement of emergency management at the trauma site by the lay population... Introducción: La avulsión dentaria se caracteriza por el desplazamiento total del diente fuera del alvéolo como consecuencia de un traumatismo externo y las medidas de urgencia en el lugar del accidente son fundamentales para un buen pronóstico. Objetivo: Indagamos el nivel de conocimientos y actitudes de los legos sobre posibles comportamientos ante la avulsión de dientes permanentes. Métodos: La recolección de datos se realizó mediante un cuestionario que contenía preguntas sobre las características generales de la lesión y las actitudes que tomarían los pacientes en caso de avulsión. Los datos fueron analizados mediante la prueba de Mann-Whitney, considerando un nivel de significación del 5% (p<0,05). Resultados: Los cuestionarios fueron respondidos por 120 pacientes en espera de atención. La media global de aciertos/ actitudes esperadas fue de 3,74. Los participantes que tenían información previa sobre el tema tuvieron promedios significativamente más altos, lo mismo no se verificó con aquellos que tenían experiencia previa con el trauma. Conclusión: Se concluye que el conocimiento sobre la avulsión dentaria de los pacientes es bajo y que la educación en salud, a través de la información poblacional, puede ser un factor relevante para la mejora del manejo de la emergencia en el sitio del trauma por parte de la población no especializada... RESUMEN Introducción: existen múltiples técnicas quirúrgicas para tratar la inestabilidad del complejo atlantoaxial. La fijación con tornillos transarticulares C1-2 (técnica de Magerl) ha mostrado altos índices de fusión, y puede ser combinada con los sistemas de alambrado descritos inicialmente, para garantizar mayor estabilidad al constructo. Presentación de caso: masculino, 62 años, que luego de una caída de dos metros, es traído a la institución con dolor cervical y con imposibilidad para mover las extremidades. Al examen neurológico, presentaba un grado B en la American Spinal Injury Classiffication Scale. Se diagnostica fractura odontoidea conminuta y luxación atloaxoidea. Se le aplica tracción cervical con lo que se logra la alineación vertebral. Se planifica tratamiento quirúrgico mediante técnica de Magerl. Ante trayectoria subóptima del tornillo inicial, se combina con fusión atloaxoidea posterior, mediante técnica de Gallie. Se mantuvo inmovilización externa durante dos semanas. Al egreso hospitalario, el paciente logra la deambulación, aunque con disparesia braquial con predominio distal. En estudios radiológicos evolutivos, no se observa fallo del sistema de instrumentación. Conclusiones: en este caso la utilización de la técnica de Gallie, ante la malposición de uno de los tornillos transarticulares C1-2, permitió conservar la estabilidad del constructo.


ABSTRACT Introduction: Multiple surgical techniques exist to treat instability of the atlantoaxial complex. Transarticular C1-2 screw fixation (Magerl's technique) has shown high fusion rates, and can be combined with the initially described wiring systems to guarantee greater stability to the construct. Case presentation: male, 62 years old, who after a two-meter fall, was brought to the institution reporting cervical pain and inability to move the limbs. On neurological examination, he had a grade B on the American Spinal Injury Classiffication Scale. A comminuted odontoid fracture and atloaxial dislocation were diagnosed. Cervical traction was applied and vertebral alignment was achieved. Surgical treatment was planned using Magerl's technique. Given the suboptimal trajectory of the initial screw, it was combined with posterior atloaxoid fusion, using Gallie technique. External immobilization was maintained for two weeks. At hospital discharge, the patient achieved ambulation, maintaining brachial dysparesis with distal predominance. In evolutionary radiological studies, no failure of the instrumentation system was observed. Conclusions: In our case, the use of the Gallie technique, in view of the malposition of one of the C1-2 transarticular screws, allowed preserving the stability of the construct.

ABSTRACT

RESUMEN: Objetivo: Sintetizar los resultados de estudios epidemiológicos sobre Traumatismo Dentoalveolar (TDA) en la población chilena. Material y métodos: Se realizó una revisión sistemática de la literatura para identificar estudios poblacionales a nivel nacional, regional, provincial y comunal, además de datos de servicios dentales de atención primaria y secundaria sobre TDA en Chile. Resultados: Se incluyeron 2 estudios con representatividad nacional y 8 realizados en centros de atención en salud. A nivel nacional, la prevalencia de TDA a los 6 años fue de un 2.57%, y a los 12 años, 4.97%. En los estudios realizados en centros de salud, el diagnóstico más prevalente en dentición primaria fueron las lesiones de tejidos de soporte y en dentición permanente, la fractura coronaria. Fue más frecuente la afectación de un solo diente, y los incisivos centrales superiores fueron los dientes más afectados. Conclusiones: Los estudios representativos de la población en Chile son escasos, realizados hace más de una década y representativos sólo de la población de 6 y 12 años, lo cual pone de manifiesto la necesidad de mayor información epidemiológica sobre el TDA en la población chilena.


ABSTRACT: Aim: To summarize the evidence on epidemiological studies about traumatic dental injuries (TDI) in Chile. Methods: A systematic literature review was carried out in two databases to identify population studies at national, regional, provincial and community levels, as well as reports from primary and secondary dental care services, regarding TDI in Chile. Results: Two studies with national representation and eight studies from primary and secondary healthcare centers were included in the analysis. National prevalence for 6-year-old children was 2.57%, and 4.97% for 12-year-old children. While the most frequent diagnosis in primary dentition was traumatic injury involving tooth-supporting tissues, the most commonly reported diagnosis in permanent dentition was crown fracture. Single tooth affection was more frequent, and the upper central incisors were the most affected teeth. Conclusions: Data on representative studies about TDIs in Chile are scarce. Few studies, carried out more than a decade ago and only on 6- and 12-year-old children are available. There is a need for further epidemiological information about TDIs in Chile.

ABSTRACT

Abstract To investigate the genetic association in a sample of replanted teeth, it is necessary to observe the extreme phenotypes, such as, teeth that underwent functional healing and those extracted due to severe external root resorption. Thus, this study aimed to investigate the association of age of the patients, root development, storage media, and polymorphisms in the interleukin 4 (IL4) and interleukin 6 (IL6) genes with teeth that presented extreme outcomes, as functional healing or extraction, in a group whose replantation techniques did not follow the International Association of Dental Traumatology (IADT) 2012 guidelines. Forty-three avulsed and replanted teeth that did not follow IADT 2012 guidelines and underwent functional healing or were extracted were included. Periapical radiographs employed for this study were taken soon after tooth replantation and after 1 year. For genotypic IL4 and IL6 genes analysis, DNA of oral mucosa cells was extracted. Real-time- PCR performed for genotyping polymorphisms in IL4 and IL6 genes. Clinical and genetic variables were analyzed by the Chi-square test and the "Z" test. P values < .05 were considered significant. The results showed that functional healing and extraction were associated with storage media and with the rs2243268 of IL- 4 gene polymorphisms. As conclusion, the C rs2243268 allele of IL4 gene may have a positive relationship with functional healing teeth that were replanted not following the 2012 IADT guidelines. Keeping the tooth dry is associated to a fast loss of avulsed and replanted teeth after 1-year follow-up.


Resumo Para investigar a influência genética em uma amostra de dentes reimplantados, é necessário observar os fenótipos extremos, como os dentes que sofreram cura funcional e os extraídos devido à reabsorção radicular externa severa. Assim, este estudo teve como objetivo investigar a associação da idade dos pacientes, desenvolvimento radicular dos dentes, assim como o meio de transporte até o reimplante e polimorfismos nos genes da interleucina 4 (IL4) e da interleucina 6 (IL6) com dentes que apresentaram cura funcional ou extração, em um grupo cujas técnicas de reimplante não seguiu a International Association of Dental Traumatology (IADT) 2012. Foram incluídos 43 dentes avulsionados e reimplantados que não seguiram as diretrizes do IADT, e tiveram cura funcional ou foram extraídos. As radiografias periapicais utilizadas para este estudo foram feitas logo após o reimplante dentário e após 1 ano. Para a análise genotípica dos genes IL4 e IL6, foi extraído DNA de células da mucosa oral. PCR em tempo real realizou a análise dos polimorfismos dos genes. As variáveis ​​clínicas e genéticas foram analisadas pelos testes Qui-quadrado e "Z". Valores de p <0,05 foram considerados significativos. Os resultados mostraram que a cura ou perda dos dentes está associada ao meio de armazenamento e polimorfismos no gene rs2243268 da IL-4. Como conclusão, o alelo C rs2243268 do gene IL4 pode ter uma relação positiva com a cura functional do dente reimplantado. Manter o dente seco está associado a uma perda rápida de dentes avulsionados e reimplantados que não seguiram o IADT 2012.

ABSTRACT

O objetivo deste estudo foi avaliar se o YouTube® pode ser considerado uma fonte de informação imediata segura para o manejo correto de uma situação de avulsão dentária evidente. Para isso, o YouTube® foi utilizado para pesquisa a partir da utilização dos termos "como tratar um dente avulsionado", "reimplante de dente avulsionado" e "avulsão dentária". Os vídeos obtidos foram submetidos a critérios de inclusão e exclusão. Os vídeos inclusos foram analisados a partir de oito tópicos variáveis que condizem com as informações adequadas sobre a sequência do manejo da avulsão dentária que independente da atuação do cirurgião dentista, baseadas nos guidelines da Associação Internacional de Traumatologia Dentária (IADT). O engajamento do vídeo e o ano de publicação também foram avaliados. Foi realizada análise descritiva a partir da frequência de repetição dos dados. Ao total, 71 vídeos sobre avulsão foram obtidos na plataforma, sendo que 39 (54,92%) apresentam os critérios de inclusão. A maioria foi publicada por profissionais (74,35%) e em 2021 (23,07%). Três vídeos (7,7%) apresentam todos os tópicos considerados indispensáveis no manejo correto da avulsão, correspondendo ao padrão ouro desejado a ser obtido durante a busca no YouTube®. "Encaminhar imediatamente ao cirurgião dentista" apresenta-se como recomendação em todos os vídeos. Todos os vídeos apresentaram informações verídicas, mesmo na incompletude dos 8 tópicos. A plataforma apresenta poucos vídeos sobre a sequência completa do manejo da avulsão. Mesmo havendo veracidade em todos os vídeos, a ausência de sequer um tópico compromete o prognóstico. O predomínio de conteúdos com ausência de tópicos infere que o YouTube® é insuficiente em vídeos referentes às condutas a serem seguidas em um caso de avulsão dentária evidente que estão de acordo com os tópicos baseados no guideline da IADT, mesmo com a ausência de informações falsas sobre o manejo da avulsão dentária(AU)


The aim of this study was to evaluate whether YouTubeTM can be considered a reliable source of immediate information for the correct management of an evident tooth avulsion setting. YouTubeTM was searched using the terms 'how to deal with an avulsed tooth', 'replantation of an avulsed tooth' and 'tooth avulsion'. The videos obtained were submitted to inclusion-exclusion criteria. The videos included were analyzed based on eight variable topics that are consistent with adequate information on the sequence of tooth avulsion management that is independent of a dentist's performance, based on International Association of Dental Traumatology (IADT) guidelines. Video engagement and year of publication were also evaluated. Descriptive statistics were consequently generated based on the frequency of data repetition. A total of 71 videos were analysed. 39 (54.92%) met the inclusion criteria. Most of the had been upload by professionals (74.35%) and in 2021 (23.07%). Three videos (7.7%) present all the topics considered indispensable in the correct management of avulsion, corresponding to the desired gold standard to be obtained during the search on YouTubeTM. 'Urgent dental referral' is the recommendation in all videos. All videos presented true information even in the incompleteness of the 8 topics. The platform features few videos on the complete sequence of avulsion management. Even with veracity in all videos, the absence of just one topic compromises the prognosis. The predominance of content infers that YouTube™ is insufficient in videos referring to the procedures to be followed in a case of evident tooth avulsion that are in accordance with the topics based on IADT guideline even with the absence of false information on the management of tooth avulsion(AU)

Objetivo: Avaliação histológica da influência da administração sistêmica de solução de Breu-branco, sob o reparo periodontal e pulpar de incisivos de ratos submetidos a reimplante dentário. Material e método: O composto testado foi obtido a partir do fracionamento da resina de P. heptaphyllum (Breu-branco) em coluna cromatográfica e diluído em solução de tween 80. A avulsão foi induzida no incisivo central superior direito de 33 ratos Wistar machos, divididos em três grupos: RI (Reimplante imediato), em que os dentes foram mantidos em meio seco e reimplantados com 5 minutos, RTLI (Reimplante tardio conservado em leite integral), dentes mantidos 60 minutos em leite integral, reimplantados e após os animais foram tratados por 5 dias com soro fisiológico por gavagem, e RTLI+BB (Reimplante tardio conservado em leite integral com administração de solução de breu-branco), os dentes permaneceram 60 minutos em leite integral, reimplantados e os animais foram tratados por 5 dias com breu-branco sistemicamente por gavagem. Após 60 dias foi realizada eutanásia por sobredosagem anestésica e coletada a hemi-maxila direita contendo o incisivo reimplantado. Os cortes histológicos transversais foram corados com hematoxilina-eosina para avaliação histológica em microscopia de luz. Na análise histomorfométrica foram analisadas as características do ligamento periodontal, osso alveolar, cemento, dentina e polpa. Os testes Kruskal-Wallis e post-hoc de Dunn foram utilizados para a comparação entre os grupos. Resultados: Os três grupos no geral apresentaram resultados semelhantes na maioria das variáveis analisadas, somente o grupo RTLI apresentou diferença estatística significativa menor na organização do ligamento periodontal e inflamação aguda em comparação com o grupo controle. Conclusão: A solução de breu-branco apresentou potencial para utilização como medicação sistêmica em casos de reimplante dentário tardio, por mostrar comportamento semelhante ao reimplante imediato no processo de reparo do ligamento periodontal e pulpar(AU)


Objective: Histological evaluation of the influence of systemic administration of Breubranco solution on periodontal and pulpal repair of incisors of rats submitted to dental reimplantation. After 60 days, euthanasia was performed by anesthetic overdose and the right hemi-maxilla containing the reimplanted incisor was collected. Cross-sectional histological sections were stained with hematoxylin-eosin for histological evaluation in light microscopy. In the histomorphometric analysis, the characteristics of the periodontal ligament, alveolar bone, cementum, dentin and pulp were analyzed. Kruskal-Wallis and Dunn's post-hoc tests were used for comparison between groups. Results: The three groups in general showed similar results in most of the variables analyzed, only the RTLI group showed a statistically significant lower difference in the organization of the periodontal ligament and acute inflammation compared to the control group. Conclusion: The breu-branco solution showed potential for use as a systemic medication in cases of late dental replantation, as it behaves similarly to immediate replantation in the repair process of the periodontal ligament and pulp(AU)

Abstract Objective: To evaluate the knowledge of medical and dental students regarding the emergency management of avulsed teeth. Material and Methods: This cross-sectional survey study was performed on 1841 students. Medical (n=746) and dental (n=845) students from different universities were included in this study. A questionnaire about the emergency management of avulsed teeth was conducted on the students. Also, 250 medical students from 3rd grade were included in the study and trained. The questionnaire was applied twice, before and after the training on the subject. Results: The mean correct answer scores were similar among the medical students in different grades (p>0.05). There were no statistically significant differences between the trained medical students and clinical dental students' correct answer scores, but there was a significant difference between the pre-clinical (1st-2nd-3rd years) and clinical (4th-5th years) dental students (p<0.01). Conclusion: Medical and preclinical dental students had lack of knowledge about the emergency management of avulsed teeth, while clinical dental and trained medical students were very knowledgeable. This result indicates the importance of education (AU).

Abstract Objective: To evaluate the pattern of traumatic dental injuries among children, adolescents, and adults. Material and Methods: This was a cross-sectional analytic study of children and adults who presented with dental trauma. Data utilized for this study were sociodemographic characteristics, history of the traumatic incident, presenting complaints, traumatized teeth and classification of the traumatized tissues using Ellis and Davey's classification. Results: A total of 163 participants with 307 traumatized teeth were included in the study. The participants' age ranged from 2 years to 75 years, with a mean age of 25.36 ±15.4 years. Children <10 years were the least represented (14.7%), adolescents 10-19 years accounted for 23.9%., young adults 20 to 40 years were the most represented (44.8%) and older adults >40 years (16.6%). A higher proportion of the study participants were female 83 (50.9%) and the most prevalent complaint was broken teeth (57.1%) The most prevalent aetiology of the trauma documented was fall (36.2%). The most prevalent injury type was extended crown fracture with noticeable dentinal involvement without pulp exposure. There was a statistically significant association between injury type, aetiology and age group. Conclusion: Traumatic dental injuries affect children, adolescents and adults alike. The central incisors were the most vulnerable teeth across all age groups. Age was significantly associated with the etiology of dental trauma and injury type (AU).

ABSTRACT Dental trauma is a common complication in general anesthesia, especially when using nasotracheal intubation during the classic laryngoscopy. Risk factors range from difficult intubation through difficult airways to occlusal changes in the patient, with incisors being widely affected by dental elements. Tooth avulsion is the most frequent type of trauma, followed by lateral dislocation and root fracture. Male patient, 39 years old, presented at the Emergency and Trauma Hospital in Caruaru/PE, a motorcycle accident victim with a bilateral jaw fracture. Given the need to fix fractures, the patient underwent a surgical procedure under general anesthesia. During laryngoscopy, heavy bleeding was observed in the oral cavity. After inspection, root fracture of lateral incisor and avulsion of right central incisor were noted. The avulsionated tooth was not found. An orthopedic image intensifier was used to locate the dental element in the thoracic region. However, it was not possible to determine its location. Therefore, an upper gastrointestinal (UGI) endoscopy was requested, and the dental element was in the digestive tract at the esophagus level. The tooth was removed, and the surgical procedure for osteosynthesis of bilateral mandible fracture was carried on. Dentoalveolar traumas induced by general anesthesia are frequent and require preventive measures since they can directly affect the patient's physical, economic, and medical conditions, along with anesthesiologists and maxillofacial surgeon's risk of receiving a civil lawsuit.


RESUMO O trauma dental é classificado como uma complicação comum em anestesia geral, em especial quando utilizada a intubação nasotraqueal durante o uso do laringoscópio clássico. Os fatores de risco envolvem desde dificuldade de intubação por via aérea difícil até alterações oclusais do paciente, sendo os incisivos, elementos dentários amplamente acometidos. A avulsão é o tipo de trauma mais frequente, seguido de luxação lateral e fratura radicular. Paciente do gênero masculino, melanoderma, 39 anos, apresentou-se no Hospital de Emergência e Trauma em Caruaru/PE vítima de acidente motociclístico, apresentando fratura bilateral de mandíbula. Visto a necessidade de fixação das fraturas, o paciente foi submetido a procedimento cirúrgico sob anestesia geral. Durante a laringoscopia, observou-se sangramento intenso na cavidade oral. Após inspeção, constatou-se fratura radicular do incisivo lateral esquerdo, e avulsão do incisivo central direito, sendo este último não encontrado. Com o intuito de preservar a via área, utilizou-se intensificador de imagem ortopédico, onde foi observado que o elemento dentário estava na região torácica, porém não foi possível apontar sua localização. Sendo assim, solicitou-se vídeo endoscopia, onde o elemento dentário se encontrava na via digestiva, na altura do esôfago. O mesmo foi removido e deu-se sequência ao procedimento cirúrgico de osteossíntese de fratura bilateral de mandíbula. Conclui-se que os traumas dentoalveolares induzidos por anestesia geral são frequentes e requerem condutas com finalidade preventivas, já que podem repercutir diretamente nas condições físicas, econômicas e médico-legais do paciente, bem como, processo cível dos profissionais envolvidos.

Abstract Dental trauma is considered a complication in oral health, which has attracted the attention of researchers, as its prevalence has increased in recent years. Dental trauma in primary dentition can lead to early tooth loss and impairment of permanent successor teeth, depending on the stage of development of the tooth germ. The aim of the present study was to describe a clinical case of a 4-year-old child who was referred to a pediatric dentist after dental trauma and early loss of deciduous teeth. During the anamnesis, it was observed that the upper deciduous teeth (#51 and #61) were affected by successive traumas, resulting in the early loss of the affected teeth. Nance arch device with a tube-bar system was selected to restore function and aesthetics to the child, while the eruption of the successor permanent teeth was monitored. During the follow-up period, a delay in the eruption of #21 tooth was observed, and ulectomy of the region was indicated to allow the tooth eruption and the proper alignment of the central incisors, which occurred after 5 months of the surgical procedure. Through this report it was possible to conclude that the early diagnosis and the long-term periodic control of traumatic dental lesions in the primary dentition are of fundamental importance to prevent and minimize the damages that can occur to the permanent dentition. Resumo O traumatismo dentário é considerado uma complicação à saúde bucal que vem chamando a atenção de pesquisadores, pois sua prevalência tem aumentado nos últimos anos. O trauma dentário na dentição decídua pode ocasionar a perda precoce dos dentes, e comprometimento dos dentes sucessores permanentes, dependendo do estágio de desenvolvimento do germe dentário. O objetivo do presente estudo foi descrever um caso clínico de uma criança de 4 anos de idade que foi encaminhada ao odontopediatra após traumatismo dentário e perda precoce de dente decíduo. Durante a anamnese, foi observado que os dentes decíduos incisivos superiores (#51 e #61) foram afetados por sucessivos traumas, resultando na perda precoce dos dentes afetados. O dispositivo arco de Nance com sistema do tipo tubo-barra foi selecionado para devolver função e estética à criança, enquanto a irrupção dos dentes permanentes sucessores foi acompanhada. Durante consulta de controle, um atraso na erupção do dente #21 foi observado e a ulectomia da região foi indicada para permitir a irrupção do dente e o alinhamento adequado dos incisivos centrais, que ocorreu após 5 meses do procedimento cirúrgico. Por meio do presente relato foi possível concluir que o diagnóstico precoce e o controle periódico a longo prazo das lesões dentárias traumáticas na dentição decídua são de fundamental importância para prevenir e minimizar os prejuízos que podem ocorrer à dentição permanente. Introducción: La práctica deportiva conlleva un alto riesgo de traumatismos dentoalveolares, por lo que es necesario que los entrenadores y deportistas conozcan sobre el uso adecuado de los protectores bucales y el procedimiento de urgencia ante la aparición de un trauma dental. Objetivo: Determinar el nivel de información sobre protectores bucales y avulsión dentaria en entrenadores deportivos. Métodos: Se realizó un estudio descriptivo de corte transversal, durante el mes de febrero de 2020, en la Escuela de Iniciación Deportiva Escolar Provincial Lino Salabarría Pupo de Sancti Spíritus. Para ello se seleccionó una muestra probabilística, representativa y aleatoria de 25 entrenadores de los deportes de combate y juego con pelotas, a los cuales se le aplicó una encuesta estructurada sobre protectores bucales y avulsión dentaria. Resultados: Entre los integrantes de la serie, 76 % eran universitarios y 52 % poseían más de 10 años de experiencia profesional. Asimismo, se obtuvo que 88 % recomendaran el uso del protector bucal a sus deportistas, pero la mayoría desconocía sus características, funciones y tipos, así como la conducta a seguir ante la ocurrencia de una avulsión dentaria. Conclusiones: El nivel de información sobre protectores bucales y avulsión dentaria en los entrenadores deportivos fue deficiente, lo que demostró que es necesaria la educación en dicho tema. De hecho, los estomatólogos tienen la tarea de realizar campañas para incentivar y educar en el uso de los protectores bucales y en el procedimiento correcto de urgencia si ocurriera un trauma dentoalveolar.


Introduction: The sport practice bears a high risk of dentoalveolar trauma, reason why it is necessary that trainers and sportsmen know on the appropriate use of oral protectors and the emergency procedure in view of the emergence of a dental trauma. Objective: To determine the level of information on oral protectors and dental avulsion in sport trainers. Methods: A descriptive and cross-sectional study was carried out during February, 2020, in the Lino Salabarría Pupo Provincial Initiation Sport School in Sancti Spíritus. A probabilistic, representative and random sample of 25 trainers from combat sports and ball games was selected, to whom an structured survey on oral protectors and dental avulsion was applied. Results: Among the members of the series, 76 % were university students and 52 % had more than 10 years of professional experience. Also, it was obtained that 88 % recommended the use of the oral protector to their sportsmen, but most of them ignored its characteristics, functions and types, as well as the way to behave considering the occurrence of a dental avulsion. Conclusions: The level of information on oral protectors and dental avulsion in the sport trainers was poor, what demonstrated that the education on this topic is necessary. In fact, dentists have the task of carrying out campaigns to motivate and educate in the use of oral protectors and in the correct emergency procedure in case a dentoalveolar trauma happened.

Introduction: Traumatic tooth injuries often occur in childhood. They are not life-threatening, but they can cause a number of complications that can affect the quality of life. Proper first aid can significantly facilitate further dental treatment. The aim of this research was to evaluate preschool teachers' knowledge and attitudes about dental trauma and their motivation for further education. Material and Methods: This cross-sectional study included 54 preschool teachers. It was carried out in the form of a questionnaire consisting of 23 questions regarding traumatic tooth injuries, experiences and knowledge and teachers' interest in additional education.Results: Most of the preschool teachers, 68.51%, did not witness any dental traumas during their work experience. From the teachers who witnessed dental trauma, only 11.76% gave first aid and none of them called the dentist and followed their instructions. In case of dental avulsion 72.22% would keep the avulsed tooth in a dry gauze or tissue. Even though 81.49% consider themselves uninformed about dental, all of them are interested in additional education. Conclusion: Despite the insufficient knowledge about dental trauma, its management, and first aid it is encouraging that all of the preschool teachers included in this research are motivated to educate themselves more on this subject.


Introducción: Las lesiones dentales traumáticas ocurren a menudo en la niñez. No ponen en peligro la vida, pero pueden causar una serie de complicaciones que pueden afectar la calidad de vida. Los primeros auxilios adecuados pueden facilitar significativamente el tratamiento dental posterior. El objetivo de esta investigación fue evaluar el conocimiento y las actitudes de los profesores de preescolar sobre el trauma dental y su motivación para la educación superior. Material y Métodos: Este estudio transversal incluyó a 54 maestros de preescolar. Se llevó a cabo en forma de un cuestionario que constaba de 23 preguntas sobre lesiones dentales traumáticas, experiencias y conocimientos y el interés de los profesores en la educación adicional. Resultados: La mayoría de los maestros de preescolar (68,51%) no presenció ningún traumatismo dental durante su experiencia laboral. De los profesores que presenciaron traumatismos dentales, solo el 11,76% dio primeros auxilios y ninguno llamó al dentista y siguió sus instrucciones. En caso de avulsión dental, el 72,22% mantendría el diente avulsionado en una gasa o tejido seco. A pesar de que el 81,49% se considera desinformado sobre odontología, todos están interesados en educación adicional. Conclusion: A pesar del conocimiento insuficiente sobre el trauma dental, su manejo y primeros auxilios, es alentador que todos los maestros de preescolar incluidos en esta investigación estén motivados para educarse más en este tema

Objetivo: verificar o conhecimento de acadêmicos de Odontologia de uma instituição de ensino superior da Paraíba acerca da avulsão dentária. Métodos: foi realizado um estudo transversal, no qual 64 acadêmicos responderam um formulário com perguntas objetivas relativas a dados sociodemográficos, período de formação do curso e conhecimento e condutas em casos de avulsão dentária. Foi realizada análise descritiva de frequência absoluta e relativa dos dados (SPSS, v. 20.0). Resultados: a maioria dos acadêmicos recebeu informações sobre avulsão dentária em aulas ministradas no curso (86%) e indicaria a irrigação com soro fisiológico seguida de reimplante quando da ocorrência do trauma há menos de uma hora (64,1%) e há mais de uma hora (43,8%). O tratamento endodôntico foi indicado, independentemente do tempo do dente fora do alvéolo, por 34,4% da amostra. A contenção rígida foi a mais indicada para o dente reimplantado (48,4%) e seu tempo mínimo de proservação radiográfica respondido pela maioria dos pesquisados foi de 6 meses (48,4%). Conclusões: apesar de a maioria dos acadêmicos ter recebido informações acerca da temática, o conhecimento foi considerado insuficiente em relação a condutas referentes ao reimplante dentário, indicação do tratamento endodôntico, tipo de contenção e tempo de proservação.(AU)


Objective: to verify the knowledge of dental students from a Higher Education Institution of Paraíba about dental avulsion. Methods: a cross- -sectional study was carried out, in which 64 students answered a form with objective questions regarding sociodemographic data, course period, knowledge and conduct in cases of tooth avulsion. Descriptive analysis of absolute and relative frequency of data was performed (SPSS, v. 20.0). Results: most students received information about dental avulsion during the graduation classes (86%) and would indicate irrigation with saline solution followed by reimplantation when the trauma occurred less than one hour (64.1%) and more than one hour (43.8%). Endodontic treatment was indicated, regardless of the time the tooth was out of the dental socket, according 34.4% of the sample. Rigid retention was the most indicated for the reimplanted tooth (48.4%) and six months was the minimum radiographic follow-up time answered by most students (48.4%). Conclusions: although most students have received information about the topic, the knowledge was considered insufficient in relation to some aspects of the protocols recommended for emergency care for dental avulsion, with emphasis on behaviors related to dental reimplantation, indication of endodontic treatment, type of containment and follow-up.(AU)

Abstract Introduction: the avulsion of primary teeth is a disturbing and unexpected event. Description: this report describes the clinical case of a three-year-old child who suffered an avulsion and replantation of the primary upper central incisors at the site of the injury. The guardians sought treatment for the child at the Federal University of Minas Gerais after the replantation. Four months later, the child suffered a new trauma and the replanted teeth presented advanced mobility, root resorption and fistula. The clinical conduct was extraction and rehabilitation with a fixed esthetic maintainer. Discussion: the literature describes two treatment options for avulsion of primary incisors: replantation and non-replantation. According to a recent systematic review, the difficulty in obtaining a consensus regarding the best clinical conduct is due, in part, to the scarcity of publications that present not only follow-ups with clinical success, but also with failures. The outcomes of replantation can be influenced by several factors. The time elapsed between replantation and splinting, and the new episode of trauma, negatively influenced the prognosis in the present case, leading to failure. Replantation of primary incisors is not yet evidence-based treatment. Therefore, this option must be chosen with caution and in ideal situations. It requires constant clinical and radiographic monitoring for evaluation of outcomes.


Resumo Introdução: a avulsão de dentes decíduos é um evento perturbador e inesperado. Descrição: o presente relato descreve o caso clínico de uma criança de três anos que sofreu avulsão e reimplante dos incisivos centrais superiores decíduos no local do acidente. Os responsáveis procuraram atendimento para a criança na Universidade Federal de Minas Gerais após reimplante. Quatro meses depois, a criança sofreu novo trauma e os dentes reimplantados apresentavam mobilidade avançada, reabsorção radicular e fístula. A conduta clínica foi extração e reabilitação com mantenedor estético fixo. Discussão: a literatura descreve duas opções de tratamento para avulsão de incisivos decíduos: o reimplante e o não reimplante. De acordo com revisão sistemática recente, a dificuldade de se obter um consenso sobre a melhor conduta clínica se deve, em parte, à escassez de publicações que apresentem não apenas acompanhamentos com sucesso clínico, mas também com falhas. O reimplante pode ser influenciado por vários fatores. O tempo decorrido entre o reimplante e a contenção, e o novo episódio de trauma, influenciaram negativamente o prognóstico do presente caso, levando ao insucesso. O reimplante de incisivos decí- duos ainda não é um tratamento baseado em evidências. Portanto, essa opção deve ser escolhida com cautela e em situações ideais. Requer monitoramento clínico e radiográfico para constante avaliação do desfecho.

ABSTRACT

A luxação extrusiva é caracterizada pela separação parcial ou total entre raiz e ligamento periodontal (LPD), podendo causar danos ao cemento por meio da compressão radicular contra o osso alveolar, além da ruptura do feixe vásculonervoso apical. A movimentação dentária induzida (MDI) é uma resposta de caráter inflamatório do conjunto LPD e osso alveolar, mesmas estruturas envolvidas na luxação extrusiva, a um estímulo provocado por uma força ortodôntica. Uma informação escassa na literatura é o efeito provocado pela movimentação dentária induzida em dentes que foram submetidos à luxação extrusiva (LE). O objetivo deste trabalho foi avaliar o efeito da movimentação dentária induzida sobre dentes submetidos à luxação extrusiva. Para a realização deste pesquisa, foram utilizados 40 ratos machos Wistar divididos em 4 grupos: o grupo controle (GC); o grupo Luxação Extrusiva (LE); o grupo Movimentação Dentária Induzida (MDI); e o grupo LEMDI. A LE foi exercida por uma força de 1.500Cn e a MDI por uma força de 50Cn por um período de 7 dias, após o qual os animais foram eutanasiados, as peças submetidas ao processamento laboratorial e obtidas lâminas que foram coradas por meio de hematoxilina e eosina. Foi realizada uma análise descritiva considerando-se quatro áreas ao redor da raiz mésio-vestibular: A1 região da crista óssea mesial, A2 região médio- cervical da face mesial, A3 região médio-apical da face distal e A4 região cervical da face. Em cada uma destas áreas foram observados os evento: reabsorção óssea frontal e à distância, reabsorção radicular, infiltrado infamatório e hemoragia no LPD.Os resultados mostraram que a associação entre MDI e LE provocou grande destruição óssea na crista óssea na A1, moderadas reabsorções radiculares na A3, ausência de infiltrado inflamatório em todas as áreas e hemorragia exuberante do LPD nas áreas 2 e 3. Nos demais grupos, tais observações foram ausentes ou quando presentes consideradas muito discretas. Conclui-se que, uma vez que a MDI em dentes submetidos a LE aumenta as áreas de hemorragia no LPD, provoca grande perda óssea frontal na crista alveolar, a MDI deveria ser interrompida, aguardando-se o período de reparo do LPD(AU)


Extrusive dislocation is characterized by partial or complete separation of the root and periodontal ligament (PDL), which can cause damage to the cementum by compressing the root against the alveolar bone, and rupture of the apical nervevascular bundle. Induced tooth movement (ITM) is an inflammatory response of the whole of the SMP and alveolar bone, the same structures involved in extrusive dislocation, to a stimulus caused by an orthodontic force. Little information is available in the literature on the effect caused by induced tooth movement in teeth that underwent extrusive luxation (EL). The aim of this study was to evaluate the effect of induced tooth movement on teeth submitted to extrusive dislocation. For this research 40 male Wistar rats were used, divided into 4 groups: The control group (CG); the Extrusive luxation group (EL); the Induced Tooth Movement group (ITM); and the LEMDI group. The LE was exerted by a force of 1,500Cn and the ITM by a force of 50Cn for a period of 7 days, after which the animals were euthanized, the specimens were submitted to laboratory processing and slides were obtained and stained using hematoxylin and eosin. A descriptive analysis was performed considering four areas around the mesial vestibular root: A1 mesial bone crest region, A2 mesial face midcervical region, A3 distal face mid-apical region and A4 face cervical region. In each of these areas the following events were observed: frontal and distal bone resorption, root resorption, infamatory infiltrate and hemorrhagia in the PDL. The results showed that the association between MDI and LE caused great bone destruction in the bone crest in A1, moderate root resorption in A3, absence of inflammatory infiltrate in all areas, and exuberant hemorrhage of the PDL in areas 2 and 3. In the other groups, such observations were absent or when present considered very discrete. We conclude that, since ITM in teeth submitted to EL increases the areas of hemorrhage in the PDL, causes great frontal bone loss in the alveolar ridge, ITM should be interrupted, waiting for the period of repair of the PDL(AU)
